Dean Certifications

In accordance with Federal and State regulations and in adherence to University of Florida Regulation 4.040, students are afforded the right to request a copy of their student conduct record. Please utilize the appropriate form below to submit such a request. The Dean of Students Office maintains records relating to violations of the University's Student Conduct Code and Student Honor Code, including violations of non-Title IX sexual misconduct. Therefore, the information contained in the Student Conduct Record Release will only include incidents where a student was found in violation of either of those policies and where certain sanctions were assigned in accordance with the Records section of Regulation 4.040.

The retention of student conduct records is governed by state and federal law, as well as University policy. Depending on the age of the records and the applicable records retention requirements, some records may not be available to include in the Student Conduct Record Release. The Dean of Students Office does not release student conduct records over the phone, as we are unable to verify the identity of the caller.

Please note that our office processes these requests in the order they are received and typically responds to and completes these requests in 10-15 business days. Any document requiring academic verification can be dropped off or mailed to the Office of the University Registrar located in Criser Hall.

REQUEST BY STUDENT

If you are a current or former University of Florida student, and you want to request student records for either yourself or an external entity, please use the below form to submit your request. To process your Student Record Release, you will need to log in using your student credentials and provide your UFID (Student ID Number). This is the 8 digit number found on either a physical GatorOne Card or at One.UF.edu.

REQUEST BY EXTERNAL ENTITY

Using the below form, you may request information related to a current or former University of Florida student's conduct record. Your submission through this form must include an Authorization of Release form signed by the student whose record you are requesting. Requests submitted without an Authorization of Release cannot be completed.
